What is The Consortium at Carondelet Village?

The Consortium at Carondelet Village is a collaborative group of health and wellness providers supporting area residents to live vibrant and independent lives in their homes.

What makes the Consortium special?

The Consortium at Carondelet Village is a personally designed approach that recognizes and builds on each individual's unique strengths (physical, spiritual, emotional, intellectual, vocational and social). We promote independence and wellness, and coordinate services that help manage chronic and complex conditions.

What do we do?

We inspire older adults to share their gifts with the broader community and connect to a variety of wellness resources, which include:

  • Spiritual enrichment
  • Coordinated home health care
  • Meaningful volunteer opportunities

We serve older adults living in the following St. Paul neighborhoods: Highland Park, MacGroveland, West 7th, Summit Hill, Summit University and Union Park
